MEMBERSHIP TERMS AND PAYMENT

I understand and agree to the following membership options and payment terms offered by V-FITNESS:

Membership Options:

• 6-Month Membership: $99 total

• 3-Month Membership: $149 total

• 1-Month Membership / Month-to-Month: $249 per month

Cancellation Policy: I acknowledge that if I choose a 6-month or 3-month membership and cancel my membership before the end of the contracted term, I will be responsible for paying an early cancellation fee of $200 (two hundred dollars). This fee is due immediately upon cancellation and is separate from any remaining membership balance.
